To Qualify for the CentraCare PACE program, you must be a resident of Calhoun County Michigan, AGe The mission of CentraCare is to deliver exceptional, comprehensive healthcare for frail older adults. The goal is enabling more seniors to age in place at home. To qualify to participate, you must be a resident of Calhoun County, Michigan, age 55 or older, meet nursinig home level of care guidelines as determined by State of Michigan Determination Tool, and be able to live safely in the community with support. CentraCare receives monthly payments from Medicaid and medicare to cover services for participants. Most participants pay nothing. As a CentraCare participant, all health care services are provided and arranged by your CentraCare team. PACE participants may be fully liable for the costs of medical services from an out-of-network provider or without prior authorization with the exception of emergency services.
