In a time of health care reform and an ever changing system, A&J Home Care has committed to strive to continue its standards in the health care arena. The agency provides aides to the home, to independent and assisted living facilities and to Hospitals and Nursing Homes in both counties. Our skilled RN's meet with the families to assess and evaluate each client and develops a plan of care for each and everyone specific to their needs. We pride ourselves in not only caring for a patient, but for the whole family. We educate families to the questions that need to be asked upon discharge from the facility, understanding Medicare and Medicaid and helping them find other professionals to assist them with their ever-changing needs. Families know that we are available 24 hours a day-7days a week. We have been successful in obtaining our Medicaid License in both Westchester and Putnam and in the last year have received our CHAP Accreditation achieving a standard of excellence in the industry.
