Provided by YP.comMy daughter was fitting for custom inserts and they told us 90% of the $300 was covered by insurance...THEY WERE WRONG!!! Ended up costing me $150. I would have never had them make them if I know the real cost. DO NOT TRUST THEIR COST ESTIMATE...Confirm with your insurance first.
