Canton City Health Department's Vital Statistics Division has been registering and storing all birth and death records since 1908. The division prepares and issues certified copies of birth and death records and burial permits are issued to funeral directors. Corrective affidavits for birth and death records, as well as declaration of paternity forms, are prepared at no charge. There is no charge for certified copies issued for the Veterans' Administration, and verification of births and deaths are done for various other government agencies at no cost. Most requests by mail are answered within one- to two- business days after receipt. It is located in Canton, Ohio.
