-Doctor DID NOT document the correct amount of chemotherapy treatments I received-Has recommended surgery and additional chemo without any testing to see how I was responding to treatment-Doctor ignored complaints of side effects I got from chemotherapy. No care was given to reduce side effects and manage/treat ones I got such as blisters, numbness in hands, which can be permanent if not treated, etc.-Doctors office waited more than the allotted time in Texas to send medical records after my req...
