Our home care was established in 1991, and our hospice established in 2005. Triniti Home Health & Hospice + Elim Home Care & Hospice is jointly owned by Guardian Angels Senior Services and Cassia, an Augustana/Elim affiliation. We serve people in Anoka, Carver, Dakota, Hennepin, Isanti, Mille Lacs, Ramsey, Scott, Sherburne, Washington and Wright counties in Minnesota. We are consistently named in the top 25 percent of home care agencies nationwide for our excellent outcomes. At Triniti Home Health & Hospice, the health and safety of your patients, caregivers, and family members is our number one priority.
