Provided by YP.comI know its hell to get a referal for them to accept . when i had NO insurance is no problem getting a yearly now that I have insurance its been crazy trying to get them a referral to them cant mail it cant do nothing but fax that great for some one that is in a wheel chair and no way to go fax ..smh Im mailing it and hopefully it will be up to par for them .. geez it should bt matter how they get it as long as they get it .. and funny before i never needed one all about money for Drs and such
