I like it here because all the girls and guys are very kind and nice. The nurses are very helpful. I like the manager very much, she is a pleasure to be around

The place here is very good, and has alot of activities to keep us busy. The staff working here is very good, they make me feel safe. Someone is always available when needed.

Dont be fooled by the Pavillions website,very deceiving!Mom was to be there for 2 wks to regain strength through PT.She had heart,kidney issues.While there her leg wounds got worse,heels were so dry were bleeding(was on a blood thinner)which was concerning.While visiting,my daughter had to get the nurse to address this.Skin was so dry no skin care,no hair washing,no tooth brush-no teeth brushing.Eating wasn't encouraged or assisted(she was immobile)mouth was so dry,evident lack of fluids-mouth care.I brought soup for her,the 6thfloor missing there microwave had to go to another floor to warm up,next wkd still no microwave,saw roaches on floor in dining rm.Don't try calling, never get anyone.Was left alone in her wheel chair for a lengthy period of time,was crying-wimpering,finally someone from PT came who heard her.The plan was to set her up with a dialysis ctr and for her to come home.Social services was useless,prolonging the process totally not on top of it.On 11/24 my Mom at 69 passed away,they stated cardiac arrest after 5am cares.Was fine one moment not the next,CPR and 911 but she passed away.When speaking to the nurse on duty felt rushed,confused-she hung up on me before I was finished.I called back re my Mom's belongings again felt rushed,was told to contact the on coming shifts nurse.I stated that the funeral home was contacted,that we wanted all of my Mom's things specifically cell phone and her jacket.I couldn't believe how,what they sent back.Thrown into a garbage bag, were some of her clothes,one sock,feces soiled pants,pkgs of crackers,a warm Dole fruit cup,but no cell phone or jacket.They managed to send soiled pants,a fruit cup but not what I specifically requested.What idiot packed this bag?When I called re this was told to call the nurse the next day.I opted to call the Administrator,left a message over a week ago,have yet to get a return call.I wish I knew what caused my Moms cardiac arrest, and hate that her last moments were spent there!

I never been to see this place, but phone service sucks. The receptionireceptionist never answers so recording takes you back to enter 4th extension 1410 three times. Then they hang up on you, unbelievable. Finally when I talk with my mother she's always groggy from the meds. Nurses sound so ghetto, yelling at each other when transferring a call. Supervisor never available.

Once a good, decent rehab and nursing home when it was the William Benespn Pavilion, this establishment is now horrible. They just remodeled the building at a large expense and the patients are suffering from lack of care. There are very few caring staff members and the social workers rarely follow through on complaints. My mother has had ongoing GI issues and each time she wants to go to the hospital, they initially refuse our requests to send her. Her latest hospital stay was 13 days and I came in to find her room stripped and her belongings in storage and someone else in her bed. We never received a letter or a phone call saying she was being discharged. In addition, some of her belongings were also stolen when they packed them up for storage. As far as the general care, it is awful. The food is terrible and very repetitive and the nursing staff lets alarm bells rings and frequents forgets to change bandages. I would not recommend this facility at all.

I'm not even close sorry when I say that this place is akin to what happened during the Holocaust. My Grandmother was admitted recently and thankfully was able to leave ASAP. No one should ever be treated the way my Grandmother was treated during her stay there.



My Grandmother had a lot of water buildup in her body so medication was prescribed to help her remove that excess water and therefore she had to go to the bathroom often. When my Grandmother needed to go to the bathroom, she would request for assistance but the nurses and 'assistants' there were not willing - they would just leave her there for her to wet the bed. My grandmother was not having any of that so she would just slowly take her time, UNSUPERVISED by the nurses and workers there, and use the bathroom, only to get scolded by the workers there when they find out she has gone to use the bathroom by herself.



As part of my Grandmother's stay there, she also had to go through physical therapy. On the first day of her physical therapy session, the workers wheeled her out in a wheelchair and just left her in a corridor. She waited and waited and was never attended to after she was wheeled out of her room. A long while after, she was wheeled back into her room where she waited even longer. No one ever attended her. Since she waited for so long, she eventually had to use the bathroom again, and of course, no one was there to tend to her. She tried to go to the bathroom by herself and the worker that wheeled her in back into her chair did not lock the wheelchair so my Grandmother fell! She is now covered in bruises because (1) she was never got help from the people that work there and (2) the people that work there can't perform their job properly!



My Grandmother, during several periods of her stay, was told that if she does need to wet the bed, she should wait every 2 hours. Why, you ask? How dare this facility even suggest such a thing!? I understand that the person that told my Grandmother that was trying to look out for her, but are we serious here? If you want to look out for my grandmother, help her by assisting her to the bathroom, not by telling her a 'convenient' time to wet the bed so the sheets could be changed promptly.



We took her home after her atrocious experience and when we brought her home, she was coughing non-stop. We took her to her family doctor only to find out that she contracted bronchitis from the facility!



After her atrocious experience where (1) she was told convenient times to wet the bed, (2) where she did not get assistance to go to the bathroom when requested, (3) when she ended up falling because of the incompetence of workers there, and (4) where she ended up contracting bronchitis due to, most likely, unclean practices, where do you expect that person's quality of life to be? My grandmother was the person to raise me up when I was a child - how do you expect me to react when I find out that I can't do the same for her?



I read from another review that apparently Medicare.gov rates this place with 5 stars, but those ratings are probably the result of excessive brown-nosing.



Please - for the sake of your loved ones - do not allow them to be administered to this concentration camp. Some of the patients there are just waiting for death and the workers there probably expect the same out of all the other patients. This is unacceptable.

High Quality Rehabilitation

My 96 year old mother was rehabilitated in this facility. On my periodic visits I observed a friendly and capable staff, with double the physical therapy and occupational therapy time a day than other close by rehab sites.

i was in this center for 1 week and could not wait to get out. the initial check in procedure was with a very curt individual. i was placed on a floor with like individuals - white. they have different ethnic floors - imagine! while the place is basically clean, you do not receive good care here. i called several times for a wheel chair so that i could move myself about, but i was ignored. it wasn't until my doctor came that i was able to get one. this is a terrible place.
