Provided by YP.comThis rating is all related ONLY to the office side. Surgery side wonderful, very professional in every way. Good luck getting phone calls returned as well as office manager to respond. They are very quick to take your money not so to return it. According to customer reps they are told to tell you it can take 4 to 6 weeks a bit much. As of the date this was written I still haven't received refund nor does it sound like it is correct amount. Wait and see :/
