Personal Nurse Consultants, LLC offers a boutique concierge Registered Nursing service that provides Care Management Services. Our Clients receive a personalized Health & Wellness Assessment and targeted Care Plans outlining the path to optimal health so that you can be free from the stress, frustration and worry of our fractured health care system. We strive to bridge the gaps in health care and improve the quality of life for Patients, Caregivers and Families through targeted approaches. Why every one can benefit from a Personal Nurse Consultant? Clients think: I struggle through the health care system, not fully understanding my diagnoses or required follow up care. I want to do the best that I can, but my education is given in the most stressful situations. My learning is impaired by pain, fear, confusion and worry. Am I doing this right? Our Services Care Coordination Your Personal Nurse Consultant will coordinate care between specialists, ensuring communication between all physicians, preventing duplication of tests, missed lab work, poor communication and overlooked concerns. Your Personal Nurse Consultant can also attend appointments and provide families with follow up reports. Transitions Management Your Personal Nurse Consultant assists you or your family to find high quality in home caregivers and home healthcare services for loved ones. Your Personal Nurse Consultant will also independently monitor care provided by private caregivers. Your Personal Nurse Consultant will assist in relocation to Assisted Living Facilities and other impatient facilities for loved ones. Your Personal Nurse Consultant will assist hospital discharges and attend same day surgeries. Health Navigator Your Personal Nurse Consultant assists clients in understanding all health and wellness benefits from private insurance coverage and employer programs. Your Personal Nurse Consultant will assist with obtaining pre-authorizations and pre-treatment requirements, as well as disability paperwork completion. Disease Management Your Personal Nurse Consultant educates you, caregivers and families on new or chronic disease processes, to include management, symptom monitoring, complication prevention, dietary and lifestyle changes. Medication Management and Pre/Post Surgical Education also available.
