• Conducting comprehensive physical, safety, and psychosocial assessments of older adults
  •   Developing and implementing customized plans of care based on assessment
  • Interviewing, selecting and providing oversight of in-home staff to ensure clients needs are well me
  • Anticipating client/caregiver issues to proactively address and maintain quality care
  • Facilitate medical treatment plan and pharmacy oversight
  • Communicating/documenting all activities with appropriate parties
  • Have extensive knowledge about the costs, quality, and availability of resources in their communities
  • Travel locally to attend patient medical appointments
  • Plan, coordinate, and implement all patient care, identifying and addressing all concerns
  • Act as liaison between the geriatric client, and all parties involved in case (i.e. attorney, family, and physician, etc.)
  • Complete comprehensive written monthly reports in a timely manner
  • Participate in nursing staff educational        teleconferences and meetings
