Provided by YP.comThis company sends braces without consent, have illegally billed Medicare using my SSN, which I don't know how they got(didn't use my Medicare number), and the Provider they say authorized the braces is someone I've never heard of from Hawaii, and I live in Oregon(never been to Hawaii).
