Provided by YP.comStay far away from store #1099 in Lake Charles, La. They screwed up filing my insurance and now I have to pay hundreds of dollars for their mistake. You are probably asking how that happened, well they filed with an old insurance policy even when I gave the new insurance info. Now the old insurance is billing me. I have used CVS for years and never again. They did give me an 800 number but go figure can never get through. So far I have had excellent service at Walgreens, so do yourself a favor and go there.
