Home Health Care comprises a variety of services to help you maintain your independence in the comfort of your own home. Our team of professionals can provide the following: Skilled Nursing, Wound Care, Social Work, Medication Adherence, Rehabilitation (Physical Therapy, Occupational Therapy, and/or Speech Therapy), Telemonitoring, CareLink (a devise that signals for help in an emergency), Medical Supplies, Nutrition Service, Home Health Aide Assistance and Home Care Support Services. Hospice Care strives to improve the quality of life through dignity and compassion, helping patients at the end of their lives to achieve physical and emotional comfort. Hospice provides: Skilled Nursing, Social Work, Spiritual Support, Rehabilitation (physical therapy, occupational therapy and/or speech therapy), Medical Supplies and Equipment, Pharmaceuticals for pain relief/symptom management related to diagnosis, Nutrition Service, Hospice Aide Assistance, Volunteers and Physician Services.
