The hospital operates in four locations in Illinois including Jamestown, Lebenon, Thorntown and Zionsville. Witham Memorial Hospital was established in 1915 and officially opened on February 5, 1917. Its physicians and specialists offer advanced equipment and procedures that provide residents of Boone County, Ind. with full-service medical care. Witham Health Services maintains its offices in Lebanon, Ind.
