Provided by YP.comI escaped two years ago and STILL OWE thousands for mandatory drug screens (which I passed, of course) that were sent to OUT OF NETWORK labs without my knowledge. This clinic does NOT care about you. Go somewhere else!

Provided by YP.comDr. Mocek's care has been exemplary. I transferred to Innovative Spine Care after working for over 5 years with Kaiser Permanente. I was quite satisfied with my Kaiser doctor but moved to Arkansas and needed to find another dr. At this time, while I continue to experience pain it is under control with medication and likely to improved significantly with surgery. I do agree with other reviewers about unprofessionalism of support staff. I've accepted it as an issue I can tolerate because Dr. Mocek is such a kind and effective practitioner.

To Doctor Chris Mocek and staff:
I, XXXXX request to be released from the pain contract I have signed with Dr. Chris Mocek as of July 12th, 2011.
I have found the service I received from Dr. Mocek to be adequate but not completely helpful. But that is not my main reason for leaving your clinic. See below for my three reasons for leaving:
1) On July 1st I received a reminder call that I had an appointment on July 5th with your clinic. Knowing that my work would make it impossible for me to get away the following week due to year end closing of the state books, I called your office on the 1st only to not find anyone there to answer my call. It being a holiday weekend that means I would have to wait until the 5th (the day of my appointment) to cancel. When I did finally getting a person to change appointment times I was told they canceled my appointment for that day. And then she said that because I would run out of pain meds within a week, she would schedule me for July 12th. I reluctantly agreed that she was correct on the pain meds and agreed to come on the12th. I had to fight to get time off on that Tuesday from work since this time of year is a busy time for me. I arrived at the time she told me to come only to show up and be told I did not have an appointment and that I could not be seen. The front desk person was very rude and accused me of lying and not keeping appointments and not canceling when I actually did cancel and reschedule my appointments. This is the main reason I wish to be released and go to another doctor.
2) I find your staff to be rude and not considerate of the patients needs. Whenever I have been in there I feel I have been treated as a number not as a patient with real pains. OF which I have been in constant pain since July 12th and no doctor will help me until I have a release from you.
3) But most of all I need to have a clinic that I can go to that will have more available hours that what your clinic allows. I am off every other Friday and your office is not even open for seeing people nor is there a real person there on Fridays to answer questions or change appointments if need be.
So it is today I called your office only to be told that you will not release me until I put in writing that I wish to be released from your care. This information should have been told to me two weeks ago when I called and told them I wanted to be released. I find the professionalism of your staff to be very lacking. Since this has been an ongoing problem I request release from your care effective July 12th, 2011 and all files will be forward to my new doctor.
