Lost order, apathetic manager

We use to eat here often. Tonight our order was lost and after waiting 25 minutes our order was finally prepared with no apology for the wait. My Mama Mucho Nacho order looked great until eating the top to reveal a pile of beans the size of a small tortoise. I asked the man cleaning tables if he could have the manager come to our table. He asked her twice but she never came to our table thus we will take our business elsewhere in the future. Mama Rita's had been great in the past until tonight's disaster in service and food.
