We share your commitment to help your loved one remain independent and living in the community as long as possible. The center offers a unique continuum of care, easing transitions as individual needs change. Our program is designed for adults who have medical conditions, physical disabilities, cognitive concerns, difficulties functioning at home alone, or Alzheimer’s or other memory loss conditions. Certified by the Wisconsin Department of Health & Family Services. Caring Support to Your Loved One We develop personalized care plans to tailor services and activities to individual needs, interests and abilities. Care plans, written with client and family involvement, are reviewed regularly to evaluate outcomes and goals. Highly trained staff provides health monitoring, assistance with medications, and support with feeding, toileting, mobility, and nail care. Nutritionally balanced meals and snacks are served as well. Customized Programming for Memory Loss Our day center customizes programming based on the level of memory loss and other needs. Programming is designed and adapted to maintain and encourage cognitive skills. Members receive a full day of activities to help maintain a regular daily routine and sleeping pattern. Each day’s activities are scheduled to fit the rhythms and behaviors of individual members. Tours We invite you to visit our adult day center. To scheduled a tour call Kari Chase, Human Service Specialist at (608) 663-7117. The Adult Day Center is open 7:30 a.m. – 4:30 p.m., Monday to Friday. Our Human Service Specialist can provide you local transportation options.
