Poor staffing. Frequent 12 hour shifts with only 1 person on duty. Frequent turnover, some do not get through initial training. Meal shopping & prep very poor. Many staple items frequently missing. Poor overall maintenance. Water running off roof in front of doorway & sidewalk cracked @3017. Area outside of entry is treacherous for elderly visitors. Bathroom water runs cold. Kitchen faucet broken. Hallways smell like urine. Resident rooms have spider webs & dust in the corners. No ongoing regular activities for residents. TV on constantly. No quiet music time. Staff not trained to care for dementia patients.
