Embassy of Park Avenue is a skilled nursing and rehabilitation center, located towards the end of Park Avenue Extension. We offer long-term care to many friends, family, and neighbors from the tri-county area. We also specialize in short-term rehabilitation- physical therapy, speech therapy, occupational therapy, and therapeutic modalities focused on getting patients home sooner and decreasing a return to the hospital.
