Provided by YP.comGot a packet of about a dozen new patient forms and filled out all but two. One form asked a long list of highly personal medical questions (not related to a sleep disorder) and second form was LIFETIME medical release form! Later the office manager (Jennifer) told me I had to fill them out and that I had better not "play with Medicare." impling they may not process my claim. She also threatened that she would not release the test result to the two doctors I had signed a release for. I have reported the issues to my doc and if the sleep lab doesnt immediately process my claim, I'll report them to the Florida Medical Board and Medicare.
