I will not hesitate to refer Golden Horizons to family and friends that will need the extra helping hand that i have needed. The staff and caregivers are willing to give 110% .

Service to each client is comprised of the following major components: **EVALUATION**: * A complete assessment of the needs of the elder person. **PLAN:** * A coodinated plan is written that addresses the needs identified in the elder person * Agreement on the plan is achieved by a consultaiton with the client and conccerned family members. **COMMUNICATION:** * Adjustments to the plan may be made in the early stages of care by closely monitoring the effectiviveness of the plan with the client and involved family members. **Special Care** We will also provide a similar care plan to the following. * Persons recovering from hospital stays that care facing eighter short or long-term rehabilitation at home. * 24 Hour service can be provided witha live-in companion who coordinates all aspects of daily living **Other S ervices** Golden Horizons broadens its scopes of in-home care by providing some unique advantages to the elder person such as: * Contacts, information, referrals and transporation to senior citizen activities in the local area. * Information and referrals to other types of assisted lving arrangements (adult homes, boarding homes and nursing homes.) * Referrals to elder care attorneys as required.
