Dedicated care is provided for patients with all terminal illnesses, including, but not limited to, cardiovascular disease, cancer, Alzheimer's, kidney disease, neurological disorders and AIDS. **How Does the Catholic Community Hospice Team Work?** Hospice empowers the patient and family to direct their plan of care. A registered nurse is on call and available 24 hours a day.
