Care Level Management, established in 2001, provides a wide range of health care services to elders and minorities in medically underserved communities in the United States. The center s products and services include Personal Visiting Physician delivery system, long-term maintenance, crisis intervention, home hospitalization, custodial nursing care, and pharmacy management. It also offers Medicare risk adjustment determination, care coordination, post-hospital stay follow up, post-emergency room visit follow up, skilled nursing facility, and hospitalist program. The center contracts with national health plans, such as Blue Cross, Blue Shield, Humana, Health Net, Aetna, and Cigna to provide intensive medical management for chronically ill elderly patients. It provides emergency services 24-hours a day, seven-days a week. Care Level Management is headquartered in Woodland Hills, Calif., and has offices in Phoenix, San Antonio, and San Francisco, Rancho Cucamonga, Tampa, and Melbourne, Fla.
