Dr. Mark Hastings performed my surgery, signed permanently out of work with life time restrictions. Sadly once the money train left Mark refused to back up his written life long restriction he gave me. Even with the report filing fee nope. If there was not the big dollars to be made he was done. Note he was paid big dollars on the front end, and now simple stamp, and sign paper work of his diagnosis is to much to ask.
