Provided by YP.comI am a above the knee amputee due to a Ranching accident and recently had the amputation done on June 22, 2018. I spent almost 10 years and many surgeries to try and save the leg with no success.
After having the amputation done I waited for the stitches to be removed and contacted Fit-Well at the Midvale office. They got me in right away and started the process to fit me with a prosthesis and had me walking with cutches within 5 days of the 1st visit in August. I had my 1st full time prosthesis on August 29th and was walking with a cane within a week. Now this is not the way it may work for everyone as each case is different. I was very determined to walk again so my situation went very quick, and the staff worked very hard to make my dream of walking again happen. Jen at the front desk even custom made my upper piece custom to my personality ( Jen has since made 2 more for me as well )
Scott even went as far as to come in after hours on a Saturday to adjust one of my prosthetic devices. Thank You Scott. The staff is very friendly and went out of their way for us since the 1st visit.
I highly recommend Fit-Well for their Prosthetics and their service!!! I come from Montana, and it is well worth the drive!

Provided by YP.comFrom the time the doctor ordered everything I would say it took 1-2 days. Whenever I needed advice or supplies the staff took care of me. The people they have working for them there are excellent . Thank you for everything.

Provided by YP.comThey were able to order the devices I needed. The orthotics fit my prosthetic leg so it catered to me perfectly. They made themselves very available for appointment times and follow up’s.

Provided by YP.comBEWARE OF FIT WELL PROSTHETIC & ORTHOTIC CENTER SLC UT… I went to Fit Well Prosthetic located at 50 South 900 East, #1 Salt Lake City, UT 84102, 801-364-3100, my doctor requested a custom made foot brace and a custom made foot insert. When I was at the office the doctor tried to give me an off the shelf insert, when I called him on this first he said my insurance did not authorize an custom foot insert, I assured him that my insurance company did indeed authorize a custom made foot insert and that the insurance company provided me with a copy of the authorization, the doctor changed his story to say now that the same off the shelf insert was a custom made foot insert, when I told the doctor I was smarter than that the doctor became irate and started complaining about how the insurance companies were ripping doctors off and causing doctors to lose money and how people wanted stuff for nothing. I told the doctor that this was between him and the insurance companies and not my fault that he did not have to agree to accept my insurance if he did not want to be one of their providers and that he needed to make me to custom brace and a custom insert. Then the doctor told me if it was up to him he would not accept my insurance but it was his boss that makes him except the insurance. Do not go this place, they will try to screw you and your insurance company, the doctor will treat you like a thief for using medical insurance. Very rude and unprofessional. I complained to the General Manager who did not seem to care even after I showed the General Manager that I turn on my cell phone to record the conversation with the doctor once it got heated. All the General Manager said was that he believed me. I left without getting a brace or a foot insert from FIT WELL PROSTHETIC & ORTHOTIC CENTER SLC UT and filed complaints with the medical board and my insurance company.

Provided by YP.comWhen I arrived for my appointment the doctor came in with a brace that was not a custom made brace, nor was it the AFO Blue Rocker brace that we had discussed previously, The brace was much larger than a brace that would have been ordered or made for my foot based on the measurements that the doctor had taken at the first appointment, the doctor did say that he would need to cut this brace down to fit, the brace appeared to look like something that the doctor already had, there was no packaging, the brace and did not appear to look new or look like the AFO Blue Rocker Brace sample that I had seen in my first visit. The doctor then pulled out and insert from an off the wall like package, something you would just buy from a drug store, when I asked the doctor about the insert not being custom and what about the custom insert he was going to cast for me, at first he told me that my insurance did not authorize a custom insert, I told him he was wrong and that I even received the copy of the approval letter and reminded him about the phone conversation we had about this on the phone, then the doctor changed his story and tried to tell me this was the custom insert. I explained to the doctor that I was much smarter than that and I wanted the same custom brace that he wanted me to pay the $140 cash for. The doctor got irate with me and said that was for a different custom insert; a L3020 and that my insurance approved a L3030. I then asked then why they did not request authorization for the L3020. Then the doctor became more irate and started complaining about how the insurance companies were ripping off doctors, and how people wanted stuff for nothing. Then the doctor left the room and came back with some soft mold material and had me put my foot into it to make the impression of my foot, after I put my foot in the material the doctor got very upset and said that this now is not going to work and that we would need to start the whole process over, and would have to get re-authorization from the insurance company and code it differently, again complaining on how people want stuff without having to pay for it and how they lose money on people like me. Without the doctor noticing I turn my cell phone on to record the conversation of this doctor, I then explain to the doctor that I am not at fault, I am a patient/customer and that this is between doctors and insurance companies. The doctor continue to say how doctors lose money on this kind of stuff and not just this kind of stuff but even more stuff. This doctor was very angry at me because I told him that he did not have to agree to accept my insurance if he did not want to be one of their providers and that he needed to make me to custom brace and a custom insert. Then the doctor told me if it was up to him he would not accept my insurance but it was his boss that makes him except the insurance. By this time I was very angry for how this very unprofessional doctor was treating me so I requested to speck to the General Manager, the Fit Well doctor was not happy about this, then I told the doctor that had recorded the appointment, the doctor was very angry and said it was wrong for me not to trust him. When the General Manager came in I explained everything to him. I requested that the doctor come in to confront my complaint. The General Manager went to get the doctor who must had refuse to come back in because I never saw the doctor again. The General Manager did listened to part of my recorded evidence and told me that he did not question me about the incident. I informed the General Manager that I would not be using their services,
