The first step in our process includes a personal consultation with a Care Coordinator at no charge to you. In our meeting we will: Thoroughly discuss your care needs Create a written, detailed Care Plan Work with you or your loved one to determine how to best meet your requirements Coordinate billing with insurance providers Forever Home, provides quality services to pediatric and adult clients who have been challenged with acute and chronic conditions. We are focus on providing the client and his/her family with the support necessary to give the best quality of life possible. We are servicing multi-culture, diversified clients in Oakland, Wayne, Macomb, Lapeer, Clinton, Genesee, Livingston, Midland, Roscommon, Saginaw, Saint Clair, Tuscola, Monroe and Washtenaw counties. We want to provide quality home health services which meet the needs and expectations of those injured, ill or disabled in their own homes.
