Integrity Pharmacy is more than just a pharmacy. Our staff of Pharmacist and Health Care Professionals works hand in hand with your physicians, nurses and caregivers to provide the highest quality of life for our patients by helping manage their medications. We believe in relationships that make life easier and we aim to assist our patients to their best possible health outcomes with our Medication Management Services. CONDUCT A HOME VISIT: An Integrity Pharmacy representative will visit you at your residence to collect prescription and medical information that is important for our team. We will discuss your medication needs and we will share with you what to expect from our staff. Your family or caregivers are always welcome to join our conversations to make sure they understand the program as well. The information is then sent to our pharmacy and reviewed by our pharmacists. REVIEW OF YOUR MEDICATIONS: Your Medications are continually reviewed. Each month, our clinical pharmacists review all medications that have been prescribed by each of your physicians. This review process assists in identifying interactions and ensures all medications are being filled at the accurate dose and increases the adherence of a consistent medication regimen. MANAGING YOUR PRESCRIPTIONS: Our Pharmacy staff will handle the Prescription process for you from start to finish. Our pharmacy staff works with your Doctors, Nurses and Caregivers to verify your prescriptions or transfer your existing prescriptions to Integrity Pharmacy. PREPARING YOUR CAREPACK: Our team of pharmacists prepares your CAREpack each month. Medications in the CAREpack are packaged according to the time of day your Doctor has recommended you take them. Each packet clearly lists the date, time of day and the medications that are in the packet. PROVIDING QUALITY AND SAFETY: Our process features the highest level of technology available. Each packet is scanned with our robotic technology to ensure accuracy for each packet. Our clinical pharmacist completes a final check on each order. MANAGING YOUR MONTHLY REFILLS: Our professional staff works directly with your Doctors and Nurses to update your prescriptions, and our staff will assist with obtaining refills for your prescriptions when necessary. Our staff will ask you about your medications to see if any changes to your medication have been made, if you have visited your Doctors, or have been in the hospital since our last conversation. We can assist you making a Doctor appointment if necessary. DELIVERING YOUR MEDICATIONS: Integrity Pharmacy delivers your custom CAREpack to you each month. We can also include other over the counter items, PRN medications and items such as creams, ointments, eye drops, inhalers and diabetic supplies when needed.
