I needed expensive gum treatment and asked if I could pay in a couple payments. I was told that could work. When checking out, the clerk said it was all arranged; she had all the info needed and I was good to leave. Then I received a credit card in the mail that I didn't request. I didn't fill out any application or sign anything but was signed up without my knowledge or approval. I cancelled my future appointments and told them to never contact me again.
