WHAT IS ADVERT COMMUNITY HEALTH PROJECT (ACHP) ACHP is a Medical Care Management program for eligible Children adults and seniors with physical disabilities or chronically ill, who are or may become at risk of placement in a nursing facility but wish to receive services in the community. We can also assist people currently in nursing facilities move back into the community. ACHP is designed to supplement the assistance already being provided by family, friends, neighbors and organizations. ACHP strengthens the ability of caregivers to continue in their vital role as primary support providers. ACHP participants work with a care manager to create an individualized plan of care based on comprehensive assessment of the participants healthcare needs Once the plan of care is approved by our Board of Directors, (CMT) Care Management Team, community-based services are put in place and monitored to ensure quality and effectiveness. In certain (ACHP) cases participants may hire their own employees.
